Eligibility criteria

Preparation for Adulthood service (PfA) works collaboratively across 番茄社区 to help young people transition into adulthood. We want to improve outcomes for young people by connecting them to 番茄社区 and community projects in their local area.

The 番茄社区 Adults Preparing For Adulthood (PfA) team can provide advice, information and support to young people who are aged between 14 and 25 years old, and meet one of the following criteria:

  • They have Special Educational Needs and a physical disability, a learning disability and/or autism and require support with at least one of the 4 PfA outcomes: Developing Independent Living Skills, Good Health and Wellbeing, Friendships, Relationships and 番茄社区, Life/Employment Skills (including Access to Employment)
  • They need support 番茄社区 as detailed in the eligible determination as stated in the
  • They have left the care system, are over 21 years old and are not in full time education
  • They have experienced trauma, making them more vulnerable into adulthood and require support to achieve a more independent future and prevent the need for longer term support
  • They are at risk of offending behaviours due to childhood experiences or trauma
  • They are a young offender, under 25 years old, who has completed their sentence

Be aware that this suitability list helps the PfA Prevention team identify whether a young person may be suitable for support from our service.

If they would like to work towards any of the 4 outcome areas, it is essential that young people want to work with the team. However, if it is likely there are eligible needs under the Care Act, the PfA Support Planning team may still be able to work with the younger person.
